Cancelled Licensee may not practice pharmacy in Texas.
Delinquent License is delinquent and may not actively engage in the practice of pharmacy until license becomes active.
Inactive Licensee may not practice pharmacy in Texas until certain requirements are met.
Probation Licensee may practice pharmacy in Texas. May be required to work under certain conditions. Contact the Board office.
Restricted Contact the Board office for information regarding practice restrictions.
Retired Licensee may not practice pharmacy in Texas.
Revoked Licensee may not practice pharmacy in Texas.
Surrender Licensee may not practice pharmacy in Texas.
