Texas Prescription Monitoring Program
Effective June 1, 2019, ALL official prescription forms ordered prior to September 1, 2018, will no longer be valid.
Exception: If a prescription is issued (written) prior to June 1, 2019, the prescription is valid for 21 days. See Board rule 315.3.

Currently, the average processing time to verify, print, mail, and receive the Official (CII) Prescription Forms is approximately 2 to 3 weeks. Please note this is only an estimate and processing times may exceed 3 weeks during periods of high order volume.
There is no expedited Official (CII) Prescription Form ordering process, and all orders are processed in the order received.
Failure to provide all validations will result in the cancellation of the order. No refund will be provided.
