  • You will not be able to submit applications, renew licenses, or make payments during this time.

Replacement Pharmacist Documents

Effective September 2019, lost or destroyed documents may be printed directly from the Board's website without a fee unless requesting a replacement hand-inscribed wall certificate.

The Texas State Board of Pharmacy allows pharmacists to print a free copy of a replacement document, including a license renewal certificate, pocket card, and/or preceptor certificate

The Board will replace, under certain conditions, a pharmacist’s hand-inscribed certificate. A fee is required to replace the hand-inscribed wall certificate.
