Change of Rank Application for a Resident Pharmacy (opens email application)

Request a Change of Rank application for pharmacies physically located in Texas. (i.e., Class A, A-S, B, C, C-S, D, F or G)

Change of Rank Application for a Non-Resident Pharmacy (opens email application)

Request a Change of Rank application for pharmacies NOT physically located in Texas but shipping to Texas patients. (i.e., Class E or E-S)
