Information Regarding Class D Petitions

In accordance with §291.93, a Class D Pharmacy may petition the board for an Expanded Formulary or Alternate Visitation Schedule. Such petition shall be resubmitted every two years in conjunction with the application for the renewal of the pharmacy.
